What is invisible orthodontics?

The main difference between the invisible orthodontics and bracket orthodontics is that the aligners of the teeth are removable and practically invisible.

It consists of the use of invisible and removable appliances, which are made from a virtual model of the patient’s teeth after performing a virtual planning of the case by the doctor Concha Gross. The use of brackets or wires is not necessary, thus avoiding the possible discomfort that these may cause. These aligners should be worn at least 22 hours a day, that is, practically always, even when sleeping, but with the advantage that these can be removed when eating and brushing your teeth. In this way, as the patient can remove the invisible aligners to eat, the oral hygiene routine is performed in a similar way as if they were not undergoing any treatment, allowing to maintain a very high level of hygiene in the mouth without extra effort.

Types of invisible orthodontics

These are the simplest cases of orthodontics. Normally, these are used to correct recurrences of a previous orthodontic treatment, or to perform slight movements.

The treatments last approximately 2-3 months and it consists of 7 aligners. We can choose to apply this treatment on a single quadrant (upper or lower) or on both.

Designed for intermediate treatments with a duration of approximately 6-8 months. With this one, gives us the option of requesting 14 aligners, and if we want to continue improving our result after the end of the active phase, a second phase of another 14 aligners at no additional cost is included. With this option we can treat space closure, rotations, slightly open bites… any treatment that does not require a sagittal correction (in the anterior-posterior plane)

This includes most of the orthodontic treatments that we perform with invisble orthodontics, since these require more than 14 aligners (treatments lasting more than 6 months). Changes in the malocclusion can made in the 3 spatial directions no matter the complexity of the case.

For this option, allows us to obtain aligners for 5 years, which is a guarantee in case the treatment takes longer than expected, if the patient must interrupt it for health reasons, moving out of the city… or to solve possible movements that may appear once the orthodontic treatment is finished and the retainers have not been used correctly.

Process of invisible orthodontics

The step prior to starting a treatment of invisible orthodontics, is the same for any type of orthodontic treatment. An oral cleaning is carried out and we have to check if it is necessary to perform any previous general dentistry treatment.

Once we have guaranteed the oral health of the patient, we can proceed with the first of the 5 steps required for the invisible orthodontics technique.

The first step to perform an orthodontic treatment is to visit the orthodontist. After an examination and an evaluation of the needs and requirements of the patient, a scan of the patient’s mouth is carried out (intraoral scanner). Once the intraoral scanner is performed, we obtain in few seconds a real image of the inside of the mouth and a simulation of the evolution of the treatment with invisible orthodontics.

Next we will have to carry out a complete orthodontic study, consisting of taking intra and extraoral photographic records, panoramic radiograph, teleradiography and/or dental CT for adult patients or for certain cases in order to obtain as much information as possible and with the greatest precision for achieving a correct diagnosis.

It is also necessary to obtain a registration of the teeth, at Gross dentistas, thanks to the fact that we use the latest technology, we avoid having to take the conventional molds with trays and alginates, which were so annoying for the patients.

After analysing all the records, the doctor will be able to assess the orthodontic treatment that best suits you.

Once decided that the chosen technique will be invisible orthodontics, all the records previously taken will be virtually sent to the work center for the doctor to start planning your treatment.

Each case is unique, so the invisible orthodontic treatment is absolutely personalised. Once the intraoral scanner and simulation have been performed, the duration of the treatment can be calculated.

Through virtual planning together with the use of a software, the patient can see how their mouth will be changing during the treatment and how it will look once the invisible orthodontics is removed.

Once the patient is satisfied with the planning of this treatment, the molds are made.

For each virtual movement, a new aligner is made which will put pressure on the teeth. This pressure will move the teeth to the virtually designed position.

The aligner will be changed approximately every 7 days (according to the doctor’s instructions), so the patient will feel some pressure on the teeth during the first minutes, as these will be moving to the virtually planned place.

Once the treatment has started, the check-ups will be carried out approximately every 6-8 weeks.

In each check-up, the doctor will assess the evolution of the case and if it is necessary to carry out additional procedures (interproximal reduction, use of attachments, intermaxillary elastics, microscrews…). These check-ups can be staggered in time thanks to the use of Dental-Monitoring by the patient. This is an artificial intelligence system that allows the doctor to monitor the evolution through photographs that the patient can take at home prior to changing each aligner. It is the most predictable way to perform an aligner treatment, since the control by the doctor is continuous, thus reducing the risk of mismatches between your aligners and your teeth, and making it easy to adjust the treatment just in time. By doing so, the control check-ups are staggered in time. The patient will only have to come to the clinic when the doctor assesses that a check-up is necessary thanks to the dental-monitoring software. It is the ideal solution especially for patients who live far from the clinic and it is difficult for them to frequently come to appointments with the doctor.

Everything will depend on the type of movement that the each case requires, and also it is important to bear in mind that it must be the orthodontist who recommends what is best for each patient.

There are certain malocclusions that need rather a treatment with brackets and others with aligners, but in almost 90 % of cases it will depend on the patient’s preferences, since both options are normally equally recommended.

Use a brush for aligners with soft bristles (not using the same one as we use for the teeth)
We should use soap to clean these, preferably neutral pH

Do not use mouthwash or cleaning tables for protheses, as these can change the colour of the splints to opaque
